Update of AHPC SF3 forms

Dear AHPs,

The Supervisory Assessment Report SF3 form has been updated. In the revised version, an additional point has been added under  “Instructions” paragraph 6  explaining that a “competent” or “exceeding” rating would mean that the supervisee’s performance in that domain is deemed satisfactory while a “developing” rating is deemed as unsatisfactory.  

The revised version of the SF3 form for each Allied Health Professional group is now available for download on this website under the “Downloads and Forms” section.  You are required to use the revised version with immediate effect.
